Respecting Choices, Advanced Care Planning
Discussing End of Life Plans early on alleviates doubt, feelings of guilt and uncertainty. It is one of the greatest gifts you can leave for family and friends.
It is a common misconception to think that Death Doulas only work with those who are actively dying. The truth is that death and illness are a potential reality for any of us at any age or any time. The earlier we discuss our wishes the more we can live our lives fully.
I am excited to announce that I have received my national certification as a Respecting Choices® Advanced Care Plan Facilitator.
Creating an Advanced Directive can be the most difficult part of End of Life Planning. It is a document that communicates our wishes should we experience an illness or accident that leaves us unable to communicate for ourselves. Your health care proxy is your advocate in this situation.
By having an in-depth one hour long conversation with a facilitator (me), you and your health care proxy, we will address questions and concerns and craft your advanced care plan. This candid conversation will support you in clearly communicating your wishes to your health care agent so that they are prepared should you be unable to make medical decisions for yourself.
Respecting Choices® is an internationally recognized, evidence-based system for person-centered decision making that transforms the healthcare culture by honoring an individual’s goals and values. Through this facilitated conversation your health care proxy will feel more confident in making decisions for you.
